Semester and Academic Year Abroad

Complete all forms requested by the program provider (BU, ISEP, Bilateral Partner, KIIS, CC-CS….) and submit them to the appropriate office.
Enroll in the mandatory FLIS 201 Perspectives of World Culture pre-departure course for the semester prior to your study abroad experience.
Obtain course approval for the courses you plan to take abroad
Submit all required documents on the Pre-Departure Checklist by Friday, April 19 for Fall 2013/Academic Year 2012-2013 students or by Monday, November 25 for Spring 2014 students:
- Bilateral/ISEP signed Participant Placement Acceptance Form Packet
- IPO Study Abroad Participant Placement Acceptance Form
- Schedule of Fees
- Bellarmine/IPO Agreement and Release Form
- Bellarmine University IPO Emergency Contact Information Form
- Bellarmine IPO Physician's Form
- Pre-Departure Orientation Signature Form
- Proof of international health and evacuation insurance (contact IPO for more details)
- U.S. Student Health Insurance
- Student Arrival Information Sheet
- Flight Schedule
- Passport Copy
- Visa Copy
- Study Abroad Course Approval Form
- BU Long Term Study Abroad Checklist
- A copy of your ISIC card (purchase at www.statravel.com)

Once abroad, you must fill out and return the Host Institution Enrollment Form to the IPO (via fax 502.272.8067 or email). This form is VERY important. It is to ensure you are enrolled at Full-Time Status here at Bellarmine. If you are under-enrolled while abroad, you can jeoparde your future financial aid.
