Health History and Immunization Records
A pre-matriculation (prior to entering college)
immunization policy includes documentation of routine childhood
immunizations and tuberculosis screening on or after April l of the
year of admission. Hepatitis B vaccination is strongly recommended.
The forms to document this information are included in the Orientation booklets.
A Health History Form (doc) is to be completed prior to moving into the
residence halls. Commuters are not required to turn in health history
forms unless utilizing the services of the school nurse.
